Reporting an Alma Integrity Standards Violation

If you would like to formally express your concern regarding the behavior of an Alma provider you must submit this form and include a written statement as well as supplementary information in support of your statement.

Upon receipt of this form, Alma’s team will notify the provider being reported. The provider will be asked to submit a written statement and will be encouraged to submit any supplementary information supporting their statement. Then, this statement, as well as the formal complaint, will be reviewed by Alma’s Integrity Oversight Committee.

Lastly, the provider will be notified of the date on which their case will be reviewed, possible ramifications based on the policy in violation, and an estimate of when they can expect to receive the Committee’s decision.

Please note that Alma enforces the standards of each provider’s individual state licensure boards.
